This is a first-person adventure game in which players assume the role of a baby with psychic powers. As players observe Baby Isaac's family, they can solve puzzles and make action/thought/dialogue choices to further the storyline. Some actions allow players to telekinetically throw people around rooms, shock them, or cause fires; one scene gives players the choice to kill a character, though the camera fades to black. The dialogue/text sometimes references suggestive material (e.g., “I got so many girls I still gotta f--”; "Vulva Vendetta"; “Why does every girl want you?”). Dialogue also references weed/marijuana and psychedelics. The words “sh*t” and “a*shole” appear in the game.
