This is an open-world action game in which players assume the roles of characters involved in an American crime family during the 1940s and '50s. Players complete various missions to advance the storyline, most of which involve criminal activity such as stealing cars, robbing gas stations, running drugs, or killing rival gang members. Players use pistols, tommy guns, shotguns, knives, and explosives to kill enemies and occasionally civilians. Blood frequently spurts from injured characters, stains walls/floors, and occasionally splatters on the screen. Some cutscenes depict instances of intense violence: close-range execution, stabbing, throat-slitting. One scene briefly depicts a character being attacked by several men with meat cleavers. During the course of the game, players can encounter some sexual content and/or nudity: brothels with prostitutes in revealing lingerie; a gentleman's club with female dancers; Playboy magazine pin-ups with topless images and bare buttocks. One cutscene briefly depicts the top of a woman's head as she performs fellatio on a man—the camera cuts away to other patrons at the table, the man's reactions. The player's character can order alcohol at a bar and drink until the screen becomes blurry; players are able to drive their cars in this impaired state. A few characters can occasionally be seen smoking marijuana or sampling cocaine. The words “f**k,” “sh*t,” and “n**ger” can be heard in the dialogue.
