In this first-person shooter, players assume the role of William "Wild Bill" Sherman, an ex-CIA Agent who finds himself in purgatory, fighting for his soul, paying for the mistakes of missions' past. Players use guns, rotary blades, rockets, grenades, and a "stake gun" to kill giant demons, golems, and creatures with grey flesh, decaying skin—not quite human. Guns and grenades cause creatures to break apart into pieces or emit gushes of red blood; the stake gun pins creatures to walls, where they flail and scream; and the rotating blade slices them into blocky chunks, sending limbs and torsos flying across the dungeon-like settings. Thus enemies are frequently depicted bloodied, dismembered, or beheaded. The most intense instances of violence involve attacking demonic undead children (again, grey flesh and decaying skin): the swarming enemy schoolgirls have glowing red eyes and are often on fire—they can be riddled repeatedly with machine gun fire; the zombie-like boys in school uniforms carry bloody blades while attacking players—even after losing their heads, hearts, and upper chests. The game also contains some profanity (e.g., "sh*t" and "b*tch"); however, it is the fighting in hell—the blood and gore that comes with it—that accounts for the Mature rating.
